Abstract : | Background: Retained placenta is a significant cause of maternal morbidity and mortality in the developing countries. Even though intestinal injury had been reported as a compl icat ion of induced abor t ions fol lowing instrumentations through the genital tract the involvement of the small and large bowel as a complication of removal of retained placenta is a very rare occurrence. Case report: We present the case of a 30 year old unbooked Gravida 8 Para 7 plus zero 2 alive who had a spontaneous delivery of twins at a primary health centre. The delivery was complicated by a retained placenta which was removed both manually and partly with the use of instruments. This led to a posterior uterine rupture with evisceration of small bowel through the vaginal introitus. She subsequently had subtotal abdominal hysterectomy and right hemi- colectomy with ileo- colic anastomosis. Conclusion: This case indicates the need to have skilled birth attendants and early referral at our primary health centers which will help reduce maternal morbidity and mortality.
